Woolhope Secondary School Application Form 2023

Grade 8 Application forms can be collected from the front office.

Office hours: Mon – Thur (08:00 – 16:00)

Friday (08:00 – 12:00)

Please take note of the following information that must be attached to application form.

1. Certified copy of childs birth certificate
2. Certified copy of both parents/guardian or step parents ID
3. Certified copy of death certificate (if applicable)
4. Grade 7 latest report (March or June)
5. Proof of address (Muncipal Account, Lease Aggreement etc)
6. Proof of income ( 1 month bank statement or payslip)
7. Affidavit to state who will be taking responsible for school fee
8. Proof of grant ie. SASSA (if applicable)

The above mentioned documents must be attached to an application form.

Application forms must be collected at the school.

Each application forms has a number which we keep track of.

Application fee of R50 Non refundable must be paid once form has been submitted.

Applications must be submitted to the school. Submission via email, fax or any form of electronic submission will not be accepted.
