Please note that these documents must accompany this application. It is advised that you have these documents available in appropriate formats (preferably PDF or JPEG) on your computer, to upload in the sections to follow, before starting with the process. 1. Certified copy of Birth Certificate or Identity Document of scholar.
2. Certified copy of Identity Document of each parent and/or guardian.
3. Copy of latest report from the scholar’s present school. If the mid-year report is not available at the time of submission, the previous year’s December report should be submitted and the mid-year report to be forwarded as soon as it is available.
4. Scanned Medical Aid Card (both sides) as well as a copy of ID of the principle member
5. Completed Declaration by previous school: and email the Declaration Form to your previous school and have your previous school send this form DIRECTLY to WOODRIDGE for the attention of Stephanie Els via email on stephanie.els@woodridge.co.za
