This document must be completed in full using block capitals, and must be accompanied by the following:

copies of both parents ID document /passport Mother Father
copy of ID document/passport of person responsible for paying fees (if other than parent)
copy of the child’s clinic card (for Pre-School and Primary School applications only)
a recent photograph of the child (head and shoulders)
a copy of the child’s Baptism certificate (if Catholic)
a copy of the child’s most recent school report

Application Process
Please ensure that all supporting documents listed on the application form are submitted.
It is advisable to apply for admission as far in advance as possible to avoid disappointment.
Following receipt of your application form and application fee, you will be contacted by the school secretary to arrange for an
interview.
All Primary and High School applicants will be required to write an entrance assessment test.
Should the School offer your child a place, you will be required to complete an Enrolment Contract before final acceptance.
Upon final acceptance, you will be required to pay a non-refundable enrolment fee to secure your child’s place at the School.
Successful applications are subject to a positive credit rating.
The learner’s compulsory school going age for Grade 1 is the year she turns 7. St Dominic’s is an open academic institution and
admits learners of all races and religious denominations.
